Exposure and Response Prevention (ERP) is a primary treatment for obsessive compulsive disorder (OCD), particularly when intrusive thoughts and compulsive behaviours feel difficult to manage.

ERP helps people gradually face the thoughts, situations, images, objects, or sensations that trigger anxiety while reducing compulsions, rituals, avoidance, or reassurance-seeking. Over time, this can reduce fear and change the cycle that keeps anxiety going.

How ERP Therapy Works

 

ERP is based on the understanding that avoidance and compulsive behaviours can bring short-term relief but often reinforce anxiety over time.

In therapy, people are gradually and safely exposed to situations that trigger distress while learning new ways to respond.

 

This process is planned carefully and guided by each person’s goals, so it feels manageable and structured.

 

ERP is collaborative and does not involve being forced into distressing situations without preparation or consent.

When ERP therapy can be helpful

 

ERP can be helpful when you are experiencing:

  • intrusive thoughts or obsessions

  • compulsions, rituals, or checking behaviours

  • reassurance-seeking or avoidance

  • anxiety that feels difficult to manage

ERP is most commonly used in OCD treatment, and may also be used in anxiety counselling, phobias, or panic, where avoidance is maintaining distress.

What Happens in ERP Therapy

ERP therapy usually begins with understanding the situations, thoughts, or triggers that are maintaining anxiety.

Therapy may include:

  • identifying obsessions, fears, rituals, or avoidance patterns

  • understanding how anxiety is maintained over time

  • developing a gradual exposure plan

  • learning how to reduce compulsions or reassurance-seeking

  • building confidence responding differently to anxiety

  • practising strategies between sessions where appropriate

Sessions are structured and collaborative, with therapy guided by each person’s goals, pace, and readiness.
